Practice Nurse – Greenfield Medical Centre 2000 -2020
It is with deepest sadness that we announce the death of Kit Ong, our nurse practitioner and beloved colleague who passed away yesterday as the result of complications following Covid-19 infection.
Kit has been a valuable part of the Greenfield Medical Centre team since 2000. She was a loyal, committed and highly experienced nurse who brought expertise and skill to all areas of her work. She was not only caring but had a great sense of humour and was much loved by everyone here at Greenfield. She will be hugely missed by her colleagues and patients alike.
Our thoughts and prayers go out to Kit's family at this very sad time, to her colleagues and friends, and to the many patients that she devoted her time and care to over the years.
We realise that many patients may wish to pass on their own thoughts of Kit and sympathies to the family. In normal times we would have a book of condolence in reception, as this is not possible we suggest that you email us at firstname.lastname@example.org or by post direct to the surgery. We will ensure that these are then given to Kit’s family.
Covid- 19 and Greenfield Medical Centre
Due to the current situation with COVID - 19 there has been a major increase in workload for NHS Staff, including those working at Greenfield Medical Centre. We receive daily updates from NHS England and Public Health England advising us what needs to be done to align our self with national guidance and policy.
Currently we are not seeing patients face to face. All Contact is via telephone and or video link. As from Monday 6th April we will be offering e consults please read about this on the website.
Please note that our main door is closed and we will only be in contact with patients by telephone.
In order to utilise our services during this period, please ensure that you are able to access our online services. This will enable you to book telephone appointments online, check test results request medication and to access your medical requests( please note this can only be enabled with a written(email) request).
- Prescription request please do online or send an email to email@example.com. If you have not already please ensure that you nominate a pharmacy for us to send your prescriptions electronically.
- Please note We will not be taking prescription requests over the phone.
- Please do not request your prescriptions early you will not be given them
- Please do not request more medication than you usually have, this only leads to stockpiling and shortages for others. We will not be upping medication amounts.
HIGH RISK PATIENTS / SHIELDING LETTERS
Letters have been sent out for patients considered high risk. These letters have not been sent from the practice. We are reviewing the lists and where appropriate adding any patients the clinicians consider meet the specific criteria.
The next group of patients considered high risk that GPs specifically identify also need to have specific conditions. We are currently waiting further guidance and when we receive it will be sending out letters to these patients. Please do not ring us asking for a letter as we are having increasing numbers of patients who do not meet the criteria phoning. If you are considered at risk by a clinician you will receive your letter in due course.
- For the latest information about Covid-19 always refer to the NHS website (nhs.uk). Please be wary of misinformation and 'anecdotes' on social media.
EMPLOYMENT AND SUPPORT ALLOWANCE
The Government has promised that if you are sick or have been advised to self-isolate, you will be able to claim Employment and Support Allowance (ESA) from day one instead of day eight.
You can make a claim for new style Employment and Support Allowance on the Government's website. You will need to fill in a NSESAF1 claim form to apply.
The weekly payment for ESA is typically £73.10 or £57.90 for people under the age of 25.
For now the Government have relaxed the minimum income floor and have raised the payments for Universal Credit so self-employed individuals receive the same amount as someone on statutory sick pay, or £94.25 a week.
If you need to claim Universal Credit, but have COVID-19 or are self-isolating, you will now be able to make a claim and to access advance payments without needing to attend a Jobcentre Plus.
Unfortunately, the five week wait for new Universal Credit claims remains though.
BABY IMMUNISATION CLINICS
Please note that the baby immunisation clinic is by booked appointments only , please contact the surgery to book your child' s appointment, please note that we will only be allowing one child and one parent per consultation.

The practice has offered the option of email correspondence to its patients for the last few years so expect the transition to be smooth as we have had minimal traffic via our fax system.
